FREEZE OR CANCEL MEMBERSHIP

New Membership Cancellation or Freeze policy effective January 1, 2009

DAM members may freeze their membership one time per calendar year for a minimum of three months and a maximum of six months for no charge. Member must specify the start date and end date of the freeze. Requests must be submitted before the first of the month that freeze is to begin. Freezes cannot be retroactive. Membership billing will automatically restart after the freeze period unless the member submits an online cancellation request prior to the end of the freeze period. Members who submit an online request for a freeze or cancellation will receive an email confirmation from DAM confirming that request has been processed. Members may call the DAM office at 214-219-2300 or email Laurie at ldunlap@damswim.com if they wish to resume their membership prior to the end of the freeze period.

Medical freezes require a note from doctor. Mail or fax notes to DAM at 4901 Cole Ave. Dallas, TX 75205 or fax to 214-219-2320. Freeze start and end dates must be stated on letter.

Cancellations require a 30 day notice in the form of an online form submitted by the member. An Email confirmation will be sent to the member once the cancellation has been processed. Cancellations cannot be retroactive. To rejoin DAM please note that there is a $25 processing fee.
