MEMBERSHIP CANCELLATION

REGARDING FREEZES - Effective June 20, 2011, DAM will no longer offer a membership freeze option. Once you are a member and cancel your membership, you may rejoin at any time without paying the $25 enrollment fee.  Members may call the DAM office at 214-219-2300 or email Laurie at ldunlap@damswim.com with any questions.

Cancellations require a two week notice before your monthly billing date in the form of an online form submitted by the member. An email confirmation will be sent to the member once the cancellation has been processed. Cancellations cannot be retroactive. This form is a notification that you wish to cancel and this form does not automatically cancel your billing until it is processed by the office staff.
